TEACHING-SERVICE PARTNERSHIP IN THE UNIFIED HEALTH SYSTEM: IMPLEMENTATION OF OUTPATIENT PHARMACOTHERAPEUTICAL FOLLOW-UP

To describe the implementation process of the Pharmacotherapeutic Follow-up Service in a University Outpatient Clinic (UOC) of a multi-professional nature, and the variables related to the evaluation of the structure and the process. It is an experience modality report containing all steps of the deployment process. The data were collected through forms, the Integrated Health System (IHS) and the pharmacy monitoring records recorded between November 2015 and November 2016. The service was implemented through articulations between the Municipal Health Department of the city of Divinópolis-MG (SEMUSA) and the Federal University of São João del-Rei - Central West Campus (UFSJ/CCO). In this process there was the availability of a clinical pharmacist from SEMUSA and training by UFSJ/CCO. The service has an exclusive room, with the necessary supplies for the pharmaceutical consultation. The educational materials and the flow of care and referrals to the service were standardized. In the 12 months of the implementation, 312 consultations occurred, with the majority of the patients (67 %) arriving at the service by referrals from the doctors who work in the UOC, followed by referrals for primary care (22 %). The service was implemented with a structure adequate to the pharmaceutical follow-up process, and the teaching-service integration was important for its operation. However, efforts are needed to improve the process, as the number of consultations is still small. Difficulties of recognition of the service by the population and other health units of the municipality cause the flow of referral is less than the capacity of service attendance.
