Objectives: Describethe actions that were performed and analyze the results achieved with the establishment of a Pharmaceutical Hospital Service Orientation (Serviço de Orientação Farmacêutica Hospitalar, or SOFAH). Method: A descriptive and exploratory study conducted in a general public hospital, in which it was usedunivariate statistical analysis with information related to SOFAH registered in aspecific computerized spreadsheet. The analysis included the determination of absolute and relative frequencies for categorical variables, and position measurements in the case of quantitative variables. 

Results: Between March 2012 and February 2013, 97 patients were seen by SOFAH. The average age of the patients seen was 73 years. 4 (4.1%) patients were illiterate and 10 (10.3%) were dependent drinkers. The average number of drugstaken per patient in the pre-hospitalization period was 4, and 6 after being discharged from the hospital. All 97 patients treated by SOFAH received dosing guideline forms and 65 (65%) received aninformativeabout medicineaccess(InformativosobreAcesso a Medicamentos, or IAME), and they were oriented in relation to this access. We identified the need for producing boxes to organize medicines for 19 (20%) patients. Wediscovered that 79 (81%) patients were referred to Primary Health Care (AtençãoPrimária à Saúde, orAPS) using aform for pharmacotherapy referral (Ficha de encaminhamento Farmacoterapêutico, or FEF). We found that 54% of patients had 100% of their drugs prescribed at discharge, according to a municipal drug list registration (Relação Municipal de Medicamentos, or REMUME). Conclusion: The pharmaceutical service orientation at the patient’s dischargeyields increased safety for the patient, and it’s an interesting tool for strengthening the referring of a patient in the health care network.
