PHARMACOTHERAPEUTIC MONITORING PERFORMED AT HYPERTENSIVE AND DIABETIC PATIENTS AT THE HEALTH UNIT TEREZA BARBOSA: CASE STUDY SEGUIMIENTO

Arterial Hypertension and diabetes mellitus are responsible for the highest rates of morbidity and mortality in the Brazilian population and the world. Recognizing these diseases, as major public health problems, the Ministry of Health established guidelines that priories the linkage of patients to health units for treatment and monitoring. Through the Practice of Pharmaceutical Care, the pharmacist ensures the most appropriate, effective, safe and convenient therapy. This study aims to describe and evaluate the pharmacotherapeutic monitoring performed at one of the hypertensive and diabetic patients included in the practice of Pharmaceutical Care at the Health Unit Tereza Barbosa. Data were collected through interviews using structured diary cards and adapted from those used by professionals at the University of Minnesota. The patient had her pharmacotherapy evaluated, possible drug related problems detected and pharmaceutical interventions were made to resolve them. Besides the records of individual drug therapy follow letters of referral to other professionals and guidance instruments were used in writing to the user to ensure the resolution of identified drug related problems. At the end of the survey period, the user does not show adverse reaction to drugs used. The values showed a reduction of 150/120 mmHg to 120/80 mmHg, and blood glucose levels decreased from 132 mg/dL to 101 mg/dL. From this analysis it was found that the pharmacotherapeutic follow the user has enabled the promotion of health education, tackling drug related health and maintenance of the patient's therapeutic goals, and professional recognition of the pharmacist interacting with the healthcare team.
